Hybrid International Emergency Medicine (HIEM) Fellowship programme

A 4 year post-graduate Fellowship in Emergency Medicine, leading to MRCEM, FQim, FRCEM

The Hybrid International Emergency Medicine (HIEM) programme has been designed for MBBS doctors who are seeking international speciality training in Emergency Medicine as part of a four year Fellowship programme. The HIEM programme is suitable for MBBS doctors with two years’ post MBBS experience of Emergency Medicine Residency in their home country.

Two types of HIEM Fellowship programme

Hybrid International Emergency Medicine
(HIEM) International*

(with a 2 year MTI rotation to the UK NHS) 

This programme is supported by the Royal College of Emergency Medicine. Doctors who are successful on the programme will start their year 1 in their home country and start being trained with the Royal College of Emergency Medicine eportfolio.

 

*Not all doctors will meet the HIEM International (MTI) criteria, as doctors who have previously attempted the PLAB exam and who have failed will not qualify. However the HIEM National programme offers an alternative pathway.

 

Structure of the HIEM International programme

Year 0 – QiMET International will work with the Country centre to develop supervisors and mentors in the LMIC, providing training to staff at an international standard. Training includes:

– How to work with an eportoflio to evidence post graduate learning;
– How to write and produce reflective practice for e-portfolio,
– How to undertake and report an audit
– How to design and implement a quality improvement project
– How to use medical leadership to influence change and health system improvement.

 

Year 1 – weekly teaching in the home country and ARCP to review progress to year 2

Year 2 – UK NHS MTI placement

Year 3 – UK NHS MTI placement

Year 4 – home country completion of research thesis Fellowship in Quality Improvement and MRCEM exams.

 

 
The Assessment

Doctors are provided with a Clinical Supervisor and an Educational Supervisor in the LMIC and the UK and their clinical practice is assessed at the Annual Review of Competency Progression (ARCP) prior to progressing through the programme.

Hybrid International Emergency Medicine
(HIEM) National

(all three years of training are done in the home country)

The aim of the HIEM National programme is to build capability and capacity in the home country by developing the Emergency Medicine workforce.  This is a three year programme, particularly attractive for doctors who do not want to travel to the UK but would like to update their clinical and non-clinical skills to advance their career in Emergency Medicine in their home country.

Doctors will need to have an MBBS and to aspire to develop a career in Emergency Medicine as a specialty. Doctors will be trained by an international faculty for three years. This programme is supported by the Royal College of Emergency Medicine.

 
The HIEM Fellowship Programme (National Route)

Year 1 – weekly teaching in the home country and ARCP to review progress to year 2**
** If there is a placement in a UK NHS hospital, then it may be possible for trainees to switch from HIEM national to HIEM International, however this will not apply to doctors who have already undertaken PLAB previously and failed.

Year 2 – weekly teaching in the home country and ARCP to review progress to year 3

Year 3 – Final ARCP with completion of research thesis, Fellowship in Quality Improvement and MRCEM exams.
